Welcome to Home Advantage Rehab
Home Advantage Rehabilitation Solutions (HARS) was founded in 2008 by Kevin Mangan, PT, DPT and Michael Marzano, OTR/L. Together they have over 45 years combined experience in home based rehabilitation. HARS was formed when Kevin and Mike, both having extensive experience working for Certified Home Health Agencies (CHHAs), realized there was a large population of patients did not qualify for CHHA services, but were unable to attend outpatient therapy. Today, HARS has over 45 therapists serving people in their homes throughout Nassau, Suffolk, and Queens. We accept most major insurances.

Screen
Assess blood pressure, ROM, strength, balance through reliable standardized balance measures
(TUG, Tinetti and/or Berg assessments)
If deemed as increased falls risk initiate PT/OT though our falls prevention program
Fall Prevention Program
- Environmental assessment and recommendations
- Assess need for an assistive device
- Medication review and compliance
- Develop treatment plan based on deficits,including implementation of our graded progressive home exercise program
Post Fall Assessment
A history of a fall is a strong predictor of a future fall within 1 year
- Assess/investigate cause of the fall.
- If purely accidental with no increased falls risk -NO INTERVETION REQUIRED
- If now at high risk---INITIATE FALL PREVENTION PROGRAM
Continuum of Care
Traditional Model
- Fall/Medical decline ->hospital-> Rehab center->home care->small percentage go to outpatient clinic or receive outpatient services in the home
- Common problems after discharge include: tendency to be more lackadaisical, decreased compliance with meds/home exercise program/safety recommendations, and fall back into bad habits
Current Model
Fall/Medical decline ->hospital ->possible rehab cebter with limited rehabilitation due to changes in federal regulations ->home care (CHHAs) will play a significantly larger role but certification period cut in 1/2, and financial incentive to provide rehabilitation no longer exists -what impact will this have on the number of visits provided? -we can pick up where the CHHAs leave off -and can reinforce the education initiated by the CHHA- there’s lot of room for progress between no longer being homebound, or qualifying for home care services and achieving previous level of function -we can work on higher level activities and towards community reintegration. Patients do not have to be homebound to receive our services.
